Complex care should feel calm, not complicated.
When a participant has high intensity or nursing needs, preparation is everything. We take the time to understand the person, their routines, their current support plans, the risks, their preferences and communication needs, and the people around them, before support begins.
Support is then planned around the individual. Workers are prepared for the participant’s specific needs, and with the participant’s consent we coordinate with their chosen family members, carers, support coordinator and health professionals.
Before high intensity support begins, we confirm the current support plan, whether the service is the right fit, the worker training and competency required, escalation arrangements and available staffing.

With the participant's consent, we work with their health professionals and support network so care instructions, responsibilities and escalation steps are clear.
Additional participant-specific support: we also support participants with epilepsy and seizure management needs, medication-related needs and other complex arrangements, subject to the person’s current plans, worker preparation and available expertise.

When high intensity support needs to start or change
People contact us when high intensity support is needed for the first time, when health needs have changed, when hospital discharge is being planned, or when an existing arrangement is no longer working.

We talk through the participant's needs, current plans, location and preferred schedule, and what has to be prepared before support can begin.
When needs have changed
A health change, an updated plan or a new procedure can mean a fresh look at fit, worker preparation and risk arrangements. That review is normal, and we make it easy.
Transitioning home from hospital
Early contact gives us time to understand the support being asked for, speak with the right people and work out what must be ready before services begin.
Changing from another provider
We can talk through the current arrangement, notice requirements and continuity, and be honest about whether we have the availability and capability for the care involved.

Plan high intensity support before hospital discharge
Leaving hospital with high intensity or nursing needs takes real coordination. Early contact gives us time to understand the support being requested, review the plans that exist and confirm whether our team can safely provide what is needed.
With the participant’s consent, we can speak with their family or nominee, support coordinator, the hospital discharge team and the health professionals involved, so requirements and preparation are clear before the move home.
One thing we are always upfront about: we do not decide when someone is medically ready to leave hospital. Discharge decisions stay with the participant’s treating and hospital team.

Questions about high intensity NDIS supports

What are high intensity daily personal activities?
They are the daily supports that involve complex health-related procedures or higher risk, so they need extra skill. The Commission lists eight: complex bowel care, enteral feeding support, dysphagia support, tracheostomy support, ventilator support, urinary catheter support, subcutaneous injections and complex wound care.
Are you registered to provide high intensity supports?
Yes. Hope & Integrity is a registered NDIS provider, and the specific high intensity supports we can deliver are set out on our registration certificate. You can check our NDIS registration on the official Provider Register, and we confirm fit before accepting any referral.
Do workers receive participant-specific training?
Yes, and for high intensity supports it is required, not optional. Workers are prepared for the specific participant, the procedure involved, the current plan, the risks and the escalation steps, and their competency is confirmed before they provide the support independently. You are welcome to ask who has been trained and on what.
Do we need a current support or health plan?
Generally, yes. High intensity support needs a current, participant-specific plan developed or overseen by an appropriately qualified health practitioner, covering the procedure, routine, risks, responsibilities and escalation steps. If you are not sure what exists or what is missing, ask us, we will walk you through exactly what is needed.
Is community nursing the same as high intensity support?
No, and it is a fair thing to be unsure about. Community nursing is delivered by qualified nurses within their professional scope. Some high intensity activities can be delivered by trained and competent support workers where the plan and preparation allow it. Part of our job is working out, with you, which type of worker your situation needs.
Can you help someone transitioning home from hospital?
We can talk through the proposed supports before discharge and be straight with you about whether we have the capability and availability. With the participant’s consent, we can speak with the hospital team, support coordinator, family and health professionals. Discharge timing itself stays with the treating hospital team.
Can you provide behaviour support?
Where a current behaviour support plan is in place, we may be able to implement its strategies as an implementing provider, subject to fit, worker training and the required authorisations. We do not assess behaviour or develop behaviour support plans; those must come from a specialist behaviour support provider and an NDIS behaviour support practitioner.
Are these supports covered by an NDIS plan?
Funding depends on the participant’s plan, the supports approved in it and how the plan is managed. We can explain our service scope and pricing clearly, but funding itself may need confirming with the plan manager, support coordinator or the NDIA. Every plan is different, so nothing here is a promise about funding.
How quickly can high intensity support start?
Honestly, it depends: on fit, worker or nurse availability, current plans, participant-specific preparation, competency checks, funding and the service agreement. We will move as fast as the situation safely allows, and we will not promise a start date until the right safeguards are in place. That is a feature, not a delay.
What happens after I enquire?
